| Cell Phone Policy | |
Youth shouldn't bring cell phones to youth group meetings. Cell phones are not needed for these meetings. If a student does bring a cell phone, they should also bring a few dollars. Should youth use their phone in any fashion during the youth group meeting (talking, texting, holding in their hand, etc), the youth will be invited to make a $1 donation to the church. The phone is a distraction both to the owner of the phone and to all those around. As you may know, it can be difficult at times to maintain the attention of youth during youth group meetings and the youth workers appreciate everyone's support in this matter. If parents or guardians are concerned about their youth, the parents and guardians are invited to stay and observe the youth group meeting. The parents/guardians are invited to get the phone numbers of the church, senior pastor, and youth director, and contact them with concerns before, during, or after youth group meetings. All youth workers either have cell phones or quick access to communications devices should the urgent need arise to reach out for support for any matter. During special events, parents/guardians are required to provide contact phone numbers which the youth workers then keep with them during the entire event. Specific cell phone policies are given out for every event. Most events will restrict cell phone use among youth. Please respect these policies. All youth workers have cell phones for safety and communication reasons. The time to discuss these policies is not at the beginning of the event when registration is occurring, but rather several days before the event. Thank you for your cooperation. |

| Ways to Get Involved! | |
Dear Parent or Legal Guardian(s):
The care of your minor child is very important to us while he or she is away from home on youth trips and related activities. Although we pray for the safety of your child during these events, prompt medical treatment may be necessary in the event that your child would become ill or was injured. In such an occurrence - we need some basic medical information and your permission for treatment by a licensed medical doctor. Physicians and hospital personnel cannot treat your child without the consent of a parent or legal guardian unless his or her situation is life-threatening. We want to ensure that little time is lost while trying to contact you or your insurance company.
The medical release form is your consent for medical treatment in the event that your child is injured or becomes ill while participating in a youth related activity. The leaders and chaperones of West Side United Methodist Church will take these forms with them on all trips outside of the church.
This form will cover your child's participation in youth events from the date signed to August 31 of the year he/she graduates from high school. Please notify the Director of Youth Ministries or the Church Office of any changes in information during the time your child is involved in the youth program at West Side UMC.
Thank you for your cooperation.
